WHAT TO EXPECT THE DAY OF YOUR APPOINTMENT

YOUR INITIAL VISIT
To assist the doctors in your treatment, we ask that you obtain pertinent records of any tests completed in the last three years including x-rays, surgeries, and laboratory work. You will be mailed a questionnaire and other information forms. Please have this information completed prior to your appointment and please bring it with you. Also, bring your insurance cards and referrals if applicable. During the visit the doctor will review your medical history and current symptoms, as well as perform a brief physical examination. Any procedures, x-rays, or follow up appointments will be scheduled upon completion of your consultation. There is no specific preparation for your initial visit unless otherwise specified by our office. Please take medications as you normally would. |
